Eligibility criteria
Patients must meet all three of the following to be eligible:
BMI
The patient must meet one of the following:
35 kg/m² or above
OR
30 to 34.9 kg/m²
AND meets the criteria for referral to specialist overweight and obesity management services:
The underlying causes of overweight or obesity need to be assessed
The patient has complex disease states or needs that cannot be managed adequately in behavioural overweight and obesity management services, such as the extra support needs of people with learning disabilities
Less intensive management has been unsuccessful
Specialist interventions such as a very low calorie diet, surgery or certain medicines are being considered
A tolerance of 2.5 kg/m² applies to each element of the criteria for at-risk groups with higher obesity prevalence, particularly people of Black African or Caribbean and South Asian origin.
Comorbidities
At least one of the following:
Hypertension
Dyslipidaemia
Obstructive sleep apnoea
Cardiovascular disease.
Prior treatment
The patient agrees to a sustained programme of lifestyle interventions, including a reduced calorie diet and increased physical activity with advice and management.
Exclusion criteria
Under 18
Currently losing weight successfully with a dietetic or reputable evidence-based weight management intervention
Pregnant, planning pregnancy, or breastfeeding
Previously referred and left the pathway early or disengaged, seeking re-referral within 12 months
Uncontrolled hypertension, heart condition, or medical condition preventing increased activity
Untreated or unstable major mental illness
Active eating disorder
Suicide attempt within the last year
Self-harm in the past 3 months
Suicide plans in the past 3 months
Previous bariatric surgery
Known hypersensitivity to semaglutide or tirzepatide or their excipients
Known severe hypersensitivity reactions such as anaphylaxis or angioedema to another GLP-1 receptor agonist, for example liraglutide, dulaglutide, exenatide or lixisenatide
Personal or family history of medullary thyroid carcinoma (MTC)
History of multiple endocrine neoplasia syndrome type 2 (MEN2)
Current, recent (within 6 months), chronic or recurrent pancreatitis
Decompensated liver cirrhosis

A specialist service built around your patient's needs
Two pathways, both consultant-led.
We assess each patient and personalise their care.
Our specialist team includes consultant physicians, specialist dietitians, health psychologists, pharmacists, nurses and exercise specialists.
Medication pathway
Medication pathway
Consultant-led MDT. GLP-1 prescribing (Wegovy) where clinically appropriate. Dietetic, psychological, and behavioural support for up to 24 months.
Behavioural pathway
Behavioural pathway
Consultant-led MDT. Dietetics, psychology, exercise support. Structured lifestyle programme for 12 months.
