Eligibility criteria

Your patient must meet one of the following

A tolerance of 2.5 kg/m² applies to each element of the criteria for at-risk groups with higher obesity prevalence, particularly people of Black African or Caribbean and South Asian origin.


Route 1

50 kg/m² or above


Route 2

40 kg/m² or above

  • Has engaged with community weight services over a 2-year period but failed to achieve or maintain weight loss goals


Route 3

35 kg/m² or above

  • Has engaged with community weight services over a 2-year period but failed to achieve or maintain weight loss goals

  • Has one or more weight-related comorbidity from the list below

Qualifiying comorbidities

  • Established cardiovascular disease

  • Type 2 diabetes

  • Hypertension

  • Idiopathic intracranial hypertension

  • Obstructive sleep apnoea

  • Non-alcoholic steatohepatitis (NASH)


Exclusion criteria

  • Under 18

  • Currently losing weight successfully with a dietetic or reputable evidence-based weight management intervention

  • Pregnant (patients who become pregnant during the service can pause and return after the birth)

  • Previously referred and left the pathway early or disengaged, seeking re-referral within 12 months

  • Uncontrolled hypertension, heart condition, or medical condition preventing increased activity

  • Unstable or severe mental illness beyond the expertise of primary care

  • Active eating disorder

  • Suicide attempt within the last year

  • Self-harm in the past 3 months

  • Suicide plans in the past 3 months

  • Previous bariatric surgery

Our pathways

A specialist service built around your patient's needs

Two pathways, both consultant-led.

We assess each patient and personalise their care.

Our specialist team includes consultant physicians, specialist dietitians, health psychologists, pharmacists, nurses and exercise specialists.

Medication pathway

Medication pathway

Consultant-led MDT. GLP-1 prescribing (Wegovy) where clinically appropriate. Dietetic, psychological, and behavioural support for up to 24 months.

Behavioural pathway

Behavioural pathway

Consultant-led MDT. Dietetics, psychology, exercise support. Structured lifestyle programme for 12 months.
